A. You should either leave the information requested for last year blank or enter a 0 in those spaces if the form requires some information. A notation should also be made somewhere on the SLOE that this is the programs first year to write a group evaluation.

Who can create a SLOE?

Q. Who can complete a SLOE?
A. A SLOE may be completed by Emergency Medicine Faculty from an Emergency Medicine Residency Program.

Q. Please define Emergency Medicine Faculty?
A. We define Emergency Medicine Faculty as a faculty member of an Emergency Medicine residency program.

Q. I am a Clerkship Director at a site that does not have an Emergency Medicine Residency program however I have many students rotate through my program and have enough data to complete a SLOE. How do I create a SLOE?
A. Normally a SLOE can only be completed by faculty of an emergency medicine residency program, however we do realize there is a small number of Clerkship Directors who do have a large number of students rotate through their site and have enough data to complete a SLOE. Please contact the CORD office at cord@cordem.org.

SLOE Template Questions

Q. Can I have the SLOE in word format?
A. No, the new SLOE is only available on the new eSLOE website. This is the only version of the SLOE being released. This is to ensure uniformity in the SLOE and that the SLOE is being completed by the proper faculty.

Q. Can I use an old SLOE template if I have them completed earlier?
A. Each program will decide what they will or won't accept. It is in your best interest to have a new SLOE completed but this is entirely up to you.

Q. I am completing a "Group SLOE" Who should I list under "reference provided by"?
A. Please listed everyone who contributed to completing the form (ie.e James Smith, MD (PD); John Jones, MD (CD); Andrea Daniels, DO (APD)

Q. Whose signature should go at the bottom of the form? Can I have more than one person sign?
A. One representative from the listed individuals that completed the form should sign the form.

Q. When will the final version of the eSLOE website be released?
A. The final version will be available after the trial period ends on July 15, 2016.

Q. Are there any instructions for the eSLOE website?
A. Yes instructions can be found at www.cordem.org/eSLOE

Q. Is there a way to save the grade distribution information and other data that is the same for all applicants and have this auto populate on my SLOEs?
A. This is the exact purpose of the new eSLOE system. Once you have created your first SLOE this information will now auto populate for future SLOEs.

eSLOE Website & Login Questions

Q. My Login for the CORD site isn't working on the eSLOE website.
A. The login for the eSLOE website is a different login from the CORD website.

Q. How do I get a login to the eSLOE website?
A. All program directors for all emergency medicine residency programs were sent a login to the eSLOE website. Each program director has the ability to create a login for additional SLOE writers at their program. Please speak with your program director about creating your login.

Q. Can't the office just create a login for me?
A. No, the program director controls the access for their program, they know who is eligible to write a SLOE at their program. You will need to have your program director create a login for you.

Q. I am the Clerkship Director for my program, can you create a login for me?
A. All program directors for all emergency medicine residency programs were sent a login to the eSLOE website. Each program director has the ability to create a login for additional SLOE writers at their program. Please speak with your program director about creating your login.

A. Instructions to add a User to your program can be found here: Add a User to Your Program

Q. I am a medical student applying in emergency medicine, I want to make it easier on my SLOE writer to access the eSLOE. Can you create a login for my SLOE writer and give it to me or send it them?
A. No, if the person writing your SLOE is faculty for an emergency medicine residency program they can contact their program director about creating their login. If they are not faculty of an emergency medicine residency program but still feel they are eligible to complete a SLOE they need to contact the CORD office. We will not release login information or accept requests to set up an account for someone else.

Q. We are writing a group SLOE and want our entire team of editors to be able to make edits to the same SLOE. Is there a way for multiple users to edit one SLOE?
A. You will need to set up everyone on your team as a "Super User" instead of a regular "User". Super Users are able to see all the SLOEs created by a program.

Q. What is the difference between a "User" and a "Super User"?
A. A "User" Can create and write SLOEs for the program but can only see and edit their own SLOEs. A "Super User" has the same permissions as a User and can also see all the SLOEs created by the program as well as create additional user accounts.

Q. How do I download the SLOE PDF once created?
A. When looking at the list of your created SLOEs the date and time stamp is a clickable link to download your SLOE.
